	Thank you for your recent purchase. To better serve you, please fill out the below warranty information immediately following your receipt of your new product. This information will help our technical support staff with any future technical support questions. Your product’s warranty is only valid if the factory receives this information. This information must be submitted within 30 days and receiving your product from HTH or 30 days from the receipt from a distributor. Warranty details for your product are located in the back of your product’s owner’s manual. This warranty is only valid after the factory receives the above information. This information must be submitted within 30 days or receiving your product from the factory or your distributor. Failure to provide this information within this timeframe voids any and all warranties.

As the Owner of the above equipment, I have read the Owner’s Manual in its entirety and understand the operation, maintenance, inspection and safety requirements of the unit. It is my responsibility to ensure proper usage, installation and maintenance is performed according to the Owner’s Manual.
